Reporting Abuse, Neglect, or a Health/Safety Concern

The Sandusky County Board of Developmental Disabilities (SCBDD) has an administrative investigator assigned to investigations termed Major Unusual Incidents (MUI). These investigations stem from allegations and reports made that affect the health and welfare of an individual served by SCBDD.

The investigator coordinates investigations with authorities such as law enforcement and Child/Adult Protective Services when required and/or needed.

How to Report an Incident

Initial MUI reports must be made verbally by calling the Sandusky County Board Crisis/MUI Hotline at 419-307-0562.

Unusual Incident Report Log

Every independent and agency provider must maintain a log of Unusual Incidents or UIs. Both independent and agency providers must review UI logs at least monthly to ensure appropriate preventive measures are implemented and to identify patterns or trends. Reporting and tracking these incidents supports providers to understand trends and patterns to help ensure health and safety.
